Transcript Request for Graduates or Current Students
To obtain a copy of a student's official transcript, a signed Transcript Request Form must be submitted by the student or parent. (For a paper copy of this Form, call the Registrar at 651-696-1315.)
- For personal use and application to another secondary/high school - Return the form to the Registrar
- For application to summer programs, PSEO or college transfer admission - Return the form to College Counseling
- Seniors applying to college - Do not use this form; they request transcripts through their college counselor.
What information is on the SPA transcript?
The transcript is the official document of a student’s academic
performance in the Upper School (grades 9- 12). It includes semester grades
and year-end grades. Grades from UMTYMP are included. Unlike quarter grade
reports, the transcript does not include quarter grades, exam grades,
or athletic participation.
As of Fall 2011, the official transcript does NOT include results of college entrance exams such as SAT, ACT and Subject Tests. Only AP exam results of 4 or 5 will be included on the transcript. When test scores are required to support applications for first-year or transfer admission, the student must order official online score reports to be sent directly to the college from www.collegeboard.com or www.actstudent.org.
For students who transferred to SPA during the Upper School years, the
SPA transcript reports only coursework and grades earned at SPA and indicates
that the student was previously enrolled at another school. A copy of
the transcript from that school will be attached to the SPA transcript
when mailed to colleges or other academic programs.
If a student has participated in a foreign study opportunity or in one
of the junior semester away programs the SPA transcript will indicate
this, but will not include specific courses taken or grades earned while
away from SPA.
College courses taken through the Post-Secondary Enrollment Option or
other college programs are not listed on the transcript. If a transcript
from these college programs is provided, we can attach this information
to your SPA transcript when mailing to colleges or other academic programs.
